Puja/Priest Service Request
Please read before you begin.
The Temple Executive Committee(EC) and the Priest need a 30 day advance notice to approve your pooja/priest service request.
This form is only for requesting PRIVATE pooja requests. This form to be submitted after speaking to the priest. Please fill out the form and submit. You will get a confirmation email from the Temple.
***Please give us a minimum of 3 Weeks advance notice for all Services requests.***

1. Please NOTE that TEMPLE IS CLOSED ON THURSDAYS. It is also PRIEST'S DAY OFF.

*Enter 0 for In-Temple or Virtual services.
**For outside temple services please use Google Maps to calculate "to and from" total mileage for fastest route between Temple address and the Outside Venue address where the puja/service is to be performed.
